Please print the Application Form above, fill it in, sign it and send to Queen Mab’s CSM, PO Box 314, Furlong, PA 18925. Or you can take a picture of the filled in form and e-mail or text it to Tish (tishstreeten@gmail.com or 518-461-3631). Your application is not complete until this form has been signed, sent and received. You must send in an Application Form whether you pay by check, venmo, or PayPal.

To pay on-line, please click on the button below next to the share size of your choice. Don’t forget to click the Share with Shipping button if you live outside of the Hopewell area. Then print the Application Form and send it signed to Queen Mab’s CSM, or take a picture of signed application and e-mail it.
